  1. Thanks for letting us know your otherwise we wouldn’t be able to find what’s wrong with this screenshot 😅

  2. These things happen, I went into the hospital for a scan once and the nurse radiography person was very confused I didn’t have boobs.

  3. Unfortunately all the people who work in the hospitals and GP practices are human and mistakes do occasionally happen. There’s no harm made with this, it was clearly an accident. Just call your GP and mention it to them, they will fix it for you and have it reviewed internally to see where the error was made and hopefully it won’t happen again.

  4. I fractured my elbow at work once. Went to the hospital and got X-rays. Called my GP for the results. Was told I had a torn ligament in my knee.

  5. Anything goes nowadays. Perhaps you should just embrace the transition? Satire of course, lol

  6. Seems to be a few mixups lately. My Mrs went for a blood donation, and then when checking the app later, she found out she had “clinical notes about her last visit.” There’s 2 pages of notes on ED for someone totally different.

    I have checked.

    She doesn’t have ED.

  7. Clinical coder here! This is the kind of mistake that happens when you get carried away and trust the OCR to accurately match up patient records.

    Whoopsy gravy!

  8. Contact your GP and let them know something’s coded wrongly in your notes – best case scenario a coder or automatic recognition chose the wrong option (I was a medical coder for 5 years and the system we used would show one thing before you selected it – ie polyp nec – and then you’d pick it and boom, cervical polyp) but worst case someone else’s letter has been mistakenly put on your records and that’s how the entry got there.

  9. Mine says I reported myself as smoke free in the early 80s. I’m 30. If bloody hope I was smoke free at the ripe old age of -10

  10. Well the technical definition of cervix is neck so if you had one on your neck that there’s no problem.

  11. GDPR breach. If you haven’t already report this to the clinic (in writing, not call) as they have a statutory duty to report it.

  12. It will be someone else’s medical records but their GP has got your number or email as theirs. Please contact ur GP about this as it happened to me for 4 years a lad up north was getting all my personal info and meanwhile I was waiting for years to hear back about all sorts of tests I went fucking apeshit needless to say
