I am trying to fill a form to get my deceased father’s birth certificate but I don’t understand what that question means. Can anyone help?

1. How many sexual partners he had

2. What right do you have for this information/your relationship with the person or document proving you have legal right to the information

3. It is just to establish what gives you the right to ask for it (family status, by law, power of attorney)

4. What form are you using? I am not sure if you can get birth certificate of your parent
